General Information

REGISTRATION
The HEP DART 2009 conference was a sold out event. The Organizing Committee strongly suggests that you register early for HEP DART 2011 to ensure your confirmation, as registration will close when the capacity of the venue is reached. Priority will be given to persons submitting an abstract. On-line registration is available on this Conference Website. If you wish to pay by check, you may download a printable, faxable registration form.  Registrations sent via e-mail cannot be accepted.

Conference Registration Fees
Payment of the Conference registration fee includes the following:
          • Entrance into all scientific sessions, including poster area
          • Delegate bag, including abstract book and final program
          • Welcome reception following the Opening Session on Sunday, December 4, 2011
          • Breakfasts on Monday-Thursday, December 5-8, 2011
          • Refreshment breaks on Monday-Thursday
          • Lunches (2)
          • Poster Session Reception
          • Closing Dinner

Accompanying Guests
Payment of the Accompanying Guest registration fee includes the following:
          • Welcome reception on Sunday, December 4, 2011
          • Breakfasts on Monday - Thursday, December 5-8, 2011

CANCELLATION POLICIES

Please note that cancellation of registrations received or postmarked on or before October 4, 2011 will incur a US $75 administration fee.

Cancellation of registrations received or postmarked after October 4, 2011 and on or before
November 4, 2011 will incur a fee equivalent to 50% of the registration payment and 50% of the hotel deposit.

No refund of the registration payment and hotel deposit will be issued for cancellations postmarked after November 4, 2011.

Liability
The HEP DART 2011 Secretariat cannot accept liability for personal accidents, nor for loss of, or damage to private property of participants, either during or directly arising from HEP DART 2011. Participants should make their own arrangements with respect to health and travel insurance.

Letter of Invitation
Individuals who require a letter of invitation to obtain a travel visa may contact Informed Horizons directly by email at dmullett@informedhorizons.com. Please include your name, address, fax number and note requesting a letter of invitation.  

Scholarship Fund
The Organizing Committee wishes to ensure that all scientists working in the hepatitis field are able to attend this Conference to the benefit of their future research. Provided funds are obtained, some partial, full, and travel scholarship funds will be created to contribute toward the costs incurred by attending this Conference.  Post-doctoral fellows, nurses, assistant professors, underrepresented minorities, patient advocates, and residents of developing countries are encouraged to apply for these scholarships. Preference will be given to those applicants who are submitting an abstract.  Please send your request for scholarship to the conference secretariat by email to ajuodawlkis@informedhorizons.com  or fax to +1 866 534 6438. You must also provide a letter from your mentor/supervisor indicating your need for a scholarship and a copy of your passport or driver's license.

PRESS RELEASE POLICY
All data available in the abstracts are embargoed until the abstract book is distributed at the meeting.  All data orally presented is embargoed until after the presentation has been made. The Organizing Committee must approve all HEP DART-related press releases prior to distribution. Failure to do so will result in your registration being automatically rejected for future DART meetings. Please submit your press release to the Organizing Committee via the email address ajuodawlkis@informedhorizons.com no later than 72 hours prior to release date for approval.
